Qualicare was founded in 2001 with the vision of maintaining continuity of care for patients and in recognition of the benefit of coordinated healthcare in helping people stay at home and decrease their frequency of hospital visits. Qualicare now employs a team of R.N. case managers and certified caregivers who provide unparalleled and expert care.

When illness strikes, families can suddenly be faced with an overwhelming new reality; both physical and emotional issues, new medical regimes and ongoing doctors’ appointments. Live-in care may be required or more simply, someone to visit for a few hours, to help with daily activities. That’s where Qualicare comes in. Our RN Case Managers are professional, compassionate and individually focused on each family. They will make the appropriate assessment and provide options for the patient and family.

Although we are commonly asked to help in private home settings, Qualicare also plays a significant role in hospital admissions, where care is provided. As well, our involvement can accelerate a patient’s hospital discharge and ensure that their home is ready with both public and private services properly coordinated so the transfer home is successful.

We offer several service packages that will help in planning costs. Choices are based on the patient’s health, family’s needs and the level of service that best fits the circumstances. All packages include 24/7 on-call service, monitoring and support by an assigned Registered Nurse. An optional Options Assessment allows us to design a care plan that fits each unique situation. This plan can be followed independently or Qualicare services may be used to implement it.
